I understand that false or omitted information is grounds for rejection of this application or, if hired, termination — and that this application does not by itself create an employment contract.

Background & MVR authorization (FCRA, 15 U.S.C. §1681b) Disclosure. Consumer reports about you may be obtained for employment purposes. Those reports may include your motor vehicle record, your FMCSA Pre-Employment Screening Program (PSP) report and your criminal history. Authorization. I have read the disclosure above and I authorize Silk Road Transportation LLC to obtain those reports about me for employment purposes, now and throughout my employment. Drug & Alcohol Clearinghouse (49 CFR §382.703(b)). I also consent to Silk Road Transportation LLC running limited queries of the FMCSA Drug & Alcohol Clearinghouse about me, now and at least once a year while I drive for the company. A full pre-employment query needs my separate electronic consent inside the Clearinghouse itself, and if I refuse that, I cannot perform safety-sensitive functions.

Previous-employer safety & DOT testing records release (49 CFR §391.23 and §40.25) I authorize Silk Road Transportation LLC to contact the employers listed above and request my safety performance history and DOT drug & alcohol testing records from the past three years, and I authorize those employers to release that information.
